Information Literacy Instruction Request Form

Thank you for your interest in incorporating library instruction into your course. Use the form below (one page) to provide us with some preliminary information about your course and to request collaboration with a librarian on information literacy instruction for your students.

Your subject librarian will follow up with you in 1-2 business days to discuss a plan for instruction in more detail. If you have any questions related to information literacy instruction or scheduling with a librarian, please email us at libinstruct@uwlax.edu.

A note for multiple sections: Please fill out a separate form for each unique course requiring instruction. If you are teaching multiple sections of one course, you only need to fill out one form and indicate the number of sections.
